Monthly drawing: official rules

1. Sponsor
[Self Covered Insurance, owned by Access Unlimited Consulting, mailing address]. The licensed insurance agency we work with is not a sponsor and does not award prizes.
2. Who can enter
Legal residents of the 50 United States and D.C. who are 18 or older. [Excluded: employees of the sponsor and the agency, and their immediate families.]
3. How to enter
Open the Reviews tab on our website and choose one of two options. Both earn exactly the same entries.
- Leave a review if you have talked with our licensed advisor.
- Free entry: share a short story about your work, even if you have never talked with us.
Entries per month: 1 for a written submission, 3 for a video submission, and 4 for both. Limit one written and one video submission per person per month. Your star rating, and whether your review is positive or negative, has no effect on your entries or your chances.
4. Drawing period
[Each calendar month is a separate drawing. Entries received from the 1st through 11:59 p.m. Eastern on the last day of the month count for that month. The drawing takes place within [7] days after the month ends.]
5. Prizes
3 winners each month. Each receives one $100 gift card [brand/retailer]. Approximate retail value: [$100] each. Prizes are not insurance, premium payments, discounts or anything tied to an insurance policy.
6. How winners are chosen and notified
[Winners are selected at random from all eligible entries, weighted by entries earned. Winners are notified by email and must respond within [10] days, or an alternate winner is chosen. Odds depend on the number of entries received.]
7. Your review and publicity
Your submission is posted only if you check the box allowing it. We do not edit reviews to change their meaning, and we publish honest reviews regardless of rating. We may decline to post spam, offensive content, private information, or claims we cannot verify. Posted reviews note that the reviewer was entered in the drawing.
